No purchase necessary to enter. The odds of winning depend on the number of entries received. Winner is responsible for all applicable gift taxes associated with receipt of prizes. Winner must provide social security number and State ID for a 1099 tax form. This contest void where prohibited by law. Winner must present proper identification in person to claim prize and must be a Kansas or Missouri resident. Winner may be required to complete an affidavit of eligibility and liability release form or prize is void or will be awarded to an alternate winner at Anthony PHC’s option.
